Employee Life Insurance Choices:
• Basic: 1 times your pay (provided by your facility)
• Supplemental: an additional 1 to 5 times your pay
Dependent Life Insurance Choices:
• Between $5,000 and $10,000 for eligible dependent children
• Between $5,000 and $50,000 for spouse
Long-Term Disability Choices:
• 50% of pay
• 60% of pay
Flexible Spending Accounts Choices:

HCA 401(k) Plan Choices:
• After two months of service, automatically contribute 3% of pay before taxes
